A new theory to kill cancer cells

We may be able to kill cancer by depriving it of sugar. Glucose is a sugar that your body needs to function. The neurons in your brain need it in order to do their jobs. Your brain would literally not work if you don’t have enough glucose. In fact, glucose plays a major role in managing your body’s energy. If you don’t get enough carbohydrates in your diet, your body will begin to synthesize glucose in order to make up the difference.

The bad news is that your neurons are not the only cells that love glucose. It turns out that cancer cells also have a bit of a sweet tooth and cancer’s sugar cravings can be insatiable. When you are performing an intense workout, your muscles undergo a process called glycolysis and burn through glucose quickly. Cancer cells also burn through glucose as quickly as possible. When the cancer cells don’t have glucose to burn, they supplement their diet a protein called glutamine.

After understanding cancer loves glucose and glutamine, researchers hypothesized that they may be to block the import of glucose and halt the metabolism of glutamine in order to stop cancer’s growth. In a journal called Cell Chemical Biology, the discovery of a new molecule was reported. The high-potency molecule is called glutor. It blocks glucose-transport proteins and starves the cancer cells. This is the first molecule to do this, but it is the most efficient.

In the past, molecules either been low-potency or have only blocked one type of protein. In order to properly starve cancer to get the desired results, scientists perform a two-step process. In the first step, the cancer is treated with glutor to shut down its metabolism. This stage was effective in inhibiting growth in 44 cancer lines and non-cancer lines remained unaffected by the glutor. In the next step, researchers block an enzyme for metabolizing glutamine.

The combination of glutor and this blocked enzyme successfully suppressed cancer growth. This process has an obvious appeal to the doctors and patients alike, but there are plenty of obstacles in the way of applying it. The primary block is a legal roadblock. There are currently no FDA-approved drugs that target glucose and glutamine metabolism because all previous drugs were deemed too toxic for humans. So researchers need to prove this method is safe before it can be available to the public

How about Liquid Biopsy for detecting Colon Cancer

Liquid Biopsy Shows Promise for Colon Cancer

High sensitivity, specificity for cancerous, precancerous lesions

Action Points

  • Note that this study was published as an abstract and presented at a conference. These data and conclusions should be considered to be preliminary until published in a peer-reviewed journal.
SAN FRANCISCO -- A liquid biopsy detected circulating tumor cells (CTCs) associated with colorectal cancer with overall accuracy approaching 90%, according to a study reported here.
The test's accuracy varied according to the stage of the disease, including 77% sensitivity for detection of precancerous lesions in the colon. The test had sensitivity exceeding 97%, reflecting a low probability of false-positive result.

If the results hold up in additional testing, the liquid biopsy might offer another option for colorectal cancer screening, Wen-Sy Tsai, MD, of Linkou Chang Gung Memorial Hospital in Taoyuan, Taiwan, reported at the Gastrointestinal Cancers Symposium.
"There is still some reticence among patients to use stool-based tests or have an invasive exam like colonoscopy to detect colorectal cancer," Tsai said in a statement. "Our results may point to a solution for people who are reluctant to get an initial screening colonoscopy or are not compliant in returning stool-based test kits that they get from their doctors."
Several recent studies showed that more than 80% of patients who are reluctant to undergo colonoscopy would find a blood test acceptable, said co-author Ashish Nimgaonkar, MD, of Johns Hopkins University in Baltimore. Other studies showed that affordability remains the biggest obstacle to colorectal cancer screening for most people; a liquid biopsy for CTCs might cost less than $100, he added.
Inconvenience, discomfort, and cost all contribute to Americans' reluctant to undergo colorectal cancer screening, agreed Nancy Baxter, MD, PhD, an American Society of Clinical Oncology expert in gastrointestinal cancers.
"Thought this research needs more investigation, a simple, accurate blood test could help increase screening rates, which could ultimately improve detection of colorectal cancers at earlier stages when treatment is most likely to be curative," said Baxter, of the University of Toronto and St. Michael's Hospital.

The study involved 620 adult Taiwanese patients with scheduled routine colonoscopy or colonoscopy for follow-up of confirmed colorectal cancer. Colonoscopy and biopsy confirmed that 111 of the patients had adenomatous polyps and 327 had colorectal cancer, ranging from early- to late-stage disease. The remaining 182 patients had no evidence of adenomas or colon cancer and served as a comparison group.
The liquid biopsy required each patients to provide 2 mL of blood, which was analyzed by a test with proven ability to identify a single CTC within 1 billion cells from an adenomatous polyp. Results were compared with the colonoscopy findings by reviewers who had no prior knowledge of the findings of either test.
Using colonoscopy findings as the definitive result, investigators found that the blood test had an overall accuracy of 88%. The test had a sensitivity of 84.0% among the 182 study participants without adenomas or colorectal cancer, 76.6% for participants with adenomatous polyps, and 86.9% for the patients with colorectal cancer. The test results yielded a specificity of 97.3% in the cancer-free group, the polyp group, and the patients with confirmed colorectal cancer.
"We believe our high specificity results are important because a high number of false-positive results would discourage many people who are considering getting screened for colorectal cancer from doing so," said Tsai.
Performance analysis showed the test had an area under the curve of 0.87 for the control group, 0.84 for the participants with polyps, and 0.88 for the patients with colorectal cancer.

Following approval of the Cologuard stool DNA test for colorectal cancer screening, gastroenterologists and colon cancer specialists noted the test's poor performance for detecting adenomatous polyps. In the randomized comparing against fecal immunochemical testing that supported the FDA application, the DNA test detected 92% of colon cancers versus 42% of adenomatous polyps. The study also yielded a 13% rate of false-positives for the DNA test.
Moreover, as one gastroenterologist noted in an opinion piece for MedPage Today, no currently available screening test will replace colonoscopy as the gold standard for diagnosis of colorectal cancer. Even if the initial results with the liquid biopsy hold up in additional testing, the CTC test colonoscopy will remain the standard for definitive diagnosis and to guide biopsy of a tumor or removal of a polyp detected by the liquid biopsy, Nimgaonkar noted.

A Virtual Reality Tool Helps Ease Extreme Pain When Drugs Can’t

Published Feb 16, 2016

SnowWorld
By Jo Marchant, PhD, Special to Everyday Health
I’m feeling anxious as research assistant Christine Hoffer, at the University of Washington in Seattle, straps a small, black box to my foot. It will heat up for 30 seconds, she explains, simulating a burn to my skin. I give the resulting pain on my foot a score of 6 out of 10: intense and impossible to ignore.
Hoffer then fits me with noise-canceling headphones and virtual reality goggles, and I’m surrounded by shimmering walls of ice.
As I float through this virtual canyon, I can look up at the stars and down toward a sparkling river — and even spin around. Perched on icy ledges on either side are penguins, igloos, and snowmen throwing snowballs at me. I fire snowballs back. While I’m immersed in the game, Hoffer switches on the black heat box for several more 30-second bursts, but I’m having far too much fun to focus on my foot.
A few moments ago this was intensely uncomfortable; now I’m barely aware of the pain.

How Illusions Relieve Extreme Pain

SnowWorld, the virtual reality system I describe above, was developed by psychologist Hunter Hoffman, PhD, and colleagues at the University of Washington Medical Center in Seattle as a pioneering approach to reduce pain. It’s basically a souped-up distraction technique, he explains. The brain has a limited capacity for attention, so if you’re immersed in something else, you have less capacity left to experience pain. This is why music, video games, or going for a walk can all ease pain to a certain extent. But this virtual reality treatment is helping to tackle some of the most extreme pain in medicine: severe burns.
Patients with severe burns have to undergo regular sessions of agonizing wound treatment, during which dead tissue is scrubbed away, as well as physiotherapy to stretch and tear scar tissue as it forms. Even at the highest doses, drugs often can’t make these sessions bearable. But SnowWorld helps: Trials with patients from scalded children to soldiers gravely injured after being hit by an improvised explosive device in Iraq show that SnowWorld eases pain by 15 to 40 percent on top of any relief that drugs can provide. That’s significantly more effective than video games or music, and Hoffman believes immersion — the feeling of being present in this virtual world — is key. Creating the illusion that we’re in a safe place far from the agonizing therapy room reduces our awareness of the pain.

Mind Over Matter in Medicine

Virtual reality is one of several psychological approaches to pain that I investigated while writing my book Cure: A Journey Into the Science of Mind Over Body. Mindfulness meditation — being aware of your thoughts and feelings moment by moment — is another proven method. Rather than trying to distract themselves from pain, practitioners aim to acknowledge and embrace the pain. Trials show that removing the anxiety and fear associated with pain can dramatically reduce the distress it causes.
Placebos (fake pills) can also be very effective in treating pain, and in fact a significant proportion of the relief you feel after taking real painkillers is due not to their biochemical action, but to your psychological belief in the drugs. Meanwhile, simply receiving medical care from an empathetic practitioner can ease pain regardless of which pill you take. These changes aren’t all in the mind, but are underpinned by physical pathways, such as the release of endorphins.
What all these lines of research show is that our experience of pain, far from being a direct or inevitable consequence of damage to the body, is ultimately controlled by the brain. If you feel afraid or under threat, the warning signal is amplified. But if you feel safe and cared for — whether that’s achieved by meditating, taking a pill, or entering a virtual world — your brain perceives that the crisis is over and eases off the pain.
In Cure, I also explain how similar principles apply to other aspects of our conscious awareness, such as fatigue, nausea, and depression. And because the brain controls so many aspects of our physiology, these changes can also influence physiological functions, such as digestion and immune system activity.

Mind Over Matter in Medicine

In medical conditions from irritable bowel syndrome to multiple sclerosis, feeling stressed and alone amplifies your symptoms, while psychological techniques, from hypnotherapy to cognitive behavioral therapy, help reduce them.
The mind is not a miracle cure: It can’t get rid of cancer, banish an infection, or mend a broken leg. But in the right circumstances, it can dramatically ease symptoms and improve your quality of life — over and above the relief you get from drugs. The research shows that by harnessing your psychological resources, you can maximize the effectiveness of the drugs you take, and sometimes even replace them.
By the end of my SnowWorld experience, I’m no longer anxious. Instead, I feel empowered by such a clear demonstration of the role of the brain. I’ve learned that although you should always listen to pain, you don’t have to be ruled by it.

Engineered Stem Cells May Help in ALS

Possible clinical benefits of cell therapy may be explained by neuroprotective effects

author name

Action Points

  • Note that this study of neurotrophic factor-secreting stem cells for amyotrophic lateral sclerosis suggested the treatment may slow the progression of the disease.
  • Note that these studies were open-label and without an adequate control group.
Stem cells engineered to secrete neurotrophic growth factors (NTFs) may slow progression of amyotrophic lateral sclerosis (ALS), early results suggested.
In a post-hoc analysis of data from a combined phase I/II and IIa study, ALS patients who had NTF-producing mesenchymal stem cells delivered intrathecally had slowed disease progression when comparing scores on forced vital capacity and ALS Functional Rating Scale -- Revised (ALS-FRS-R) before and after transplant, reported Dimitrios Karussis, MD, PhD, of Hadassah Medical Center in Jerusalem, and colleagues in JAMA Neurology.
"To our knowledge, this is the first human experience with stem cells that have been induced under culture conditions to produce NTFs, thus bearing the potential to support neuronal survival and to modify the course of neurodegeneration in ALS," they wrote.
Preclinical studies have suggested that NTFs can extend survival of motor neurons in ALS, although giving single NTFs hasn't revealed any benefit.
A more effective approach might be direct delivery of multiple NTFs to the central nervous system (CNS), in this case through transplantation of cells that actively secrete these factors.
Karussis and colleagues developed a culture-based method for inducing mesenchymal stem cells to secrete several NTFs, including GDNF, brain-derived NTF, vascular endothelial growth factor (VEGF), and hepatocyte growth factor.
To determine the safety and possible efficacy of autologous transplant of mesenchymal stem cells induced to produce NTFs in patients with ALS, the researchers conducted a pair of open-label, proof-of-concept studies: one phase I/II, and the other a phase IIa study.
Patients with ALS were enrolled from June 2011 to October 2014 at Hadassah Medical Center in Jerusalem. In the phase I/II study, six patients with early ALS were given intramuscular injection of the stem cells, and six patients with more advanced disease were transplanted intrathecally.
In the phase IIa dose-escalating study, 14 patients with early-stage ALS received a combination of intramuscular and intrathecal transplantation of the autologous NTF-secreting stem cells.
All patients were followed for 3 months before transplant and 6 months after.
Karussis and colleagues found that the therapy was safe and well tolerated in both study phases. Most of the effects were mild and transient, and included headache, fever, vomiting, leg and back pain, and neck stiffness. There were no treatment-related serious adverse events.
In the phase I/II study, they saw improvement in the mean monthly rate of progression in terms of forced vital capacity and ALS-FRS-R in the intrathecal group, but not in the intramuscular group.
In the phase IIa study, they saw a more substantial improvement in the monthly rate of decline of ALS-FRS score and forced vital capacity in the months after stem cell transplant.
Given that only those who received intrathecal stem cells appeared to benefit, the researchers performed a post-hoc comparison between progression rate of ALS-FRS-R scores and forced vital capacity before and after treatment.
The rate of progression of the forced vital capacity was reduced (-5.1% to -1.2%/month percentage predicted forced vital capacity, P<0.04) as was the FRS-R score progression (-1.2 to 0.6 points per month, P=0.052) in the 6 months after transplant compared with the 3 months prior.
They also found that 87% of these patients were defined as responders, having at least 25% improvement in the slope of progression for either ALS-FRS-R or forced vital capacity 6 months after treatment.
"Although our study was primarily targeted to assess safety and not powered for efficacy, the data also provided some indications of clinically meaningful beneficial effects induced by the intrathecal treatment with [NTF-producing] mesenchymal stem cells," they wrote.
The study was limited by the small number of patients that were included, and by a lack of a placebo group, but an ongoing double-blind, placebo-controlled, multi-center phase II trial will yield more details, the researchers said
