Cluster Headaches: what is it? have you ever heard of it?

Have you ever heard of Cluster Headaches? The worst pain is known to humans. 15% to 22% of patients have suicidal thoughts during cluster headache attacks. Cluster headache is an exceruciatingly painful headache disorder.it is also known as “suicide headache”. Cluster headache is a rare chronic disease. It was reported as early 1745 in a textbook written by Gerard van Swieten. The term “cluster” is used as the attacks usually occur in groups or clusters. Attacks can occur on consecutive days for 6-8 weeks. And can occur at a very specific time of the day. Attacks frequently occur at night. Attacks are one-sided only and almost always the same side of the head. The duration of the attack is generally 15 minutes to 3 hours.

Cluster headache affects around 0.3% of the population. It usually develops between the ages of 20 and 40. It occurs rarely in children and is extremely rare after 70-75. Men are affected around three times more often than women. Research shows the involvement of the “hypothalamus” which is part of the brain involved in circadian rhythm (body clock). Alcohol is the only dietary trigger of cluster headaches. Strong odors may also trigger cluster headache attacks. Researchers have found a strong link between smoking and cluster headaches. Physical activity can relieve the pain of cluster headaches. Symptoms include runny nose, tearing, swelling, and redness of the eye.

Cluster headaches remain undiagnosed or misdiagnosed because of misleading symptoms such as sinus, ear, or toothache. The exact cause of cluster headaches is currently unknown. Research shows that genetics may contribute to triggering the attacks. Ther is no curative treatment available for cluster headaches. However, preventive medications are available. The most effective medication is called “Verapamil”. Oxygen therapy is also used for the treatment of cluster headaches inhaling pure oxygen effectively stops cluster headache attacks. Cluster headache is much more severe than migraines. The coexistence of migraine and cluster headache is rare. You know your disease sucks when its descriptive drawings look like death metal album covers.

the Role of Vitamin D in fightng off the coronvirus infection

Public health experts are advising people to take a daily
vitamin D supplements as the coronavirus lockdown continues
during the spring and summer seasons, many people will not
be able to spend time outdoors nd expose their skin to the sun.
vitamin D protects our body from illness and infections,
therefore it is even more critical during the current Covid-19
pandemic.

During the pandemic, people are advised to stay home which
means they could be getting less vitamin D.

In a study, researchers found that the mean level of vitamin D
was strongly associated with the number of cases and the

mortality rate. Vitamin D levels are severely low in older
people. They are the most vulnerable grou of the population
for Covid-19. Health authorities advise taking a supplement of
10mg on a daily basis, especially from October to March.

Taking vitamin D supplements throughout the year is
recommended if a person gets out of the house or lives in a
care home or when most of the skin is covered with clothes
when leaving your house. Dark-skinned people need to spend
more time outdoor too.

Researchers are conducting several studies on Vitamin D to
determine if it can directly reduce the risk of coronavirus

infection. People can benefit from vitamin D during the
pandemic as it improves their health condition if they are
deficient.

Additionally, new research suggests that people infected with
the coronavirus might show poorer outcomes if they have a

vitamin D deficiency.

Researchers from France and Spain are dong clinical trials.
Their gola is to identify a potential role for vitamin D in the
Covid-19 disease as it has anti-inflammatory effects and may
slightly reduce the body’s immune responses to viruses. This
might impact the body’s immune response to the coronavirus
as an extreme inflammatory immune response such a cytokine
storm can severely damage the lungs of those infected with the
coronavirus. Studies show that vitamin D helps the body to
fight off common colds and flu. It also reduces the risk of
auto-immune disease

Kawasaki disease

Kawasaki disease (KD), also known as Kawasaki syndrome, is an acute febrile illness of unknown etiology that primarily affects children younger than 5 years of age. The disease was first described in Japan by Tomisaku Kawasaki in 1967, and the first cases outside of Japan were reported in Hawaii in 1976.
Clinical signs include fever, rash, swelling of the hands and feet, irritation and redness of the whites of the eyes, swollen lymph glands in the neck, and irritation and inflammation of the mouth, lips, and throat.
KD is a leading cause of acquired heart disease in the United States. Serious complications include coronary artery dilatations and aneurysms. The standard treatment, intravenous immunoglobulin and aspirin, substantially decreases the development of these coronary artery abnormalities.
KD occurs worldwide, with the highest incidence in Japan, and it most often affects boys and younger children. KD may have a winter-spring seasonality, and community-wide outbreaks have been reported occasionally. In the continental United States, population-based and hospitalization studies estimate an incidence of KD ranging from 9 to 19 per 100,000 children under 5 years of age. In the year 2000, approximately 4248 hospitalizations with KD were reported among children under 18 in the US, of which 3277 (77%) were for children under 5 years of age. In 2009, the estimated number of hospitalizations with KD was 5447 (standard error [SE]=297), 4040 (SE=227) for children under 5 years of age.

Air pollution: a key contributor to Covid-19 deaths

Air pollution may be a key contributor to COVID-19 deaths. A new study showed that almost 80% of deaths across 4 countries were in the most polluted regions. The study shows that 78% of the coronavirus deaths occurred in juts 5 regions out od 66 administrative regions in Italy, Spain, France and Germany. These 5 regions were the most polluted.

The study compared NO2 levels in January and February in these regions with COVID-19 deaths recorded up to 19 March. The researchers assessed the atmospheric conditions to see where pollution was being trapped over the regions. The study found that 78% of the 4,443 deaths were in 4 regions in northern Italy and one around Madrid in Spain. These 5 regions had the worst combination of NO2 levels and airflow conditions that prevented the dispersal of airpollution away from the city. These regions were surrounded by mountains, which helps trap pollution. This is also the case in the Chinese Hubei province where the pandemic began.

The researchers examined levels of nitrogen dioxide and weather conditions that can prevent dirty air from dispersing away from a city. Nitrogen dioxide is a pollutant produced mlstly by diesel vehicles. Mny scientists have linked NO2 exposure to lung disease and other health tissues which could lead to more deaths due to COVID-19. The results show that long-term exposure to nitrogen dioxide may be one of the most important contributors to COVID-19 fatalities in these regions. The researchers said that poisoning our environment means poisoning our own body when our body experiences chroic respiratory stress its ability to defend itself from infections is reduced.

The researchers said the analysis shows a strong correlation but no causal link. It is important to explore the relationship between existing inflammatory condition and the immune responses to the coronavirus. Another team studied the role of fine particle pollution in the US. The team found that small increases in pollution levels in the year before the pandemic were associated with higher COVID-19 death rates.

drug therapy for schizophrenia

A deficiency in the brains of people with schizophrenia that could lead to the development of new drug therapies has been discovered by researchers at the RIKEN Center for Brain Science (CBS) in Japan. The results were published in Schizophrenia Bulletin.

In recent years, drug therapy for schizophrenia has come to a stand-still. Most of the drugs available for schizophrenia are based on dopamine, but they are ineffective in about one out of every three patients. “because we don’t have another angle on what causes schizophrenia. Many pharmaceutical companies are pulling out of schizophrenia-related drug development” says Takeo Yoshikawa, team leader at KIREN CBS. “ hopefully, our findings can provide the new angle with a new target for drug development.”

Although, schizophrenia is a well-known mental disorder that affects the brain, how it does so remains somewhat of a mystery. Scientists have known for some time that the brains of people with schizophrenia have less white matter than normal brains. White matter in the brain is made from oligodendrocytes special cells that wrap around the parts of neurons that carry outgoing signals, which help them communicate with each other. Characteristic symptoms of schizophrenia include hallucinations and the inability to distinguish reality from fantasy, which might originate in white matter abnormalities that cause irregular communication between neurons.

Led by Takeo Yoshikawa, the team at RIKEN CBS investigated sphingolipids, a group of lipids known to have many functions, some related to white matter. Postmortem analysis of the large white matter tract that connects the left and right sides of the brain showed a severe deficiency in S1P, a sphingolipid necessary for oligodendrocyte production. Further tests showed that although normal amounts of S1P had been produced, it was metabolized and degraded when it should not have been. “drugs that prevent S1P degradation could be particularly effective in treating schizophrenia” says the first author and postdoctoral research scientist, Kayoko Esaki.

Although the experiment sounds simple, measuring S1P levels in postmortem brains was a big challenge and required interdisciplinary expertise in chemistry--specifically, mass spectrometry—that was brought to the team by Esaki.” this was the first psychiatric study of the postmortem brain to use mass spectroscopic analysis, and our discovery would not have been possible without our newly established comprehensive technique for screening sphingolipids.” Says Yoshikawa.

After findings S1P sphingolipid deficiency in schizophrenia, the researcher examined the postmortem brains of people with bipolar disorder or major depressive disorder. They found that S1P levels did not differ from what they found in normal brains, indicating that the problem is specific for schizophrenia and not a common feature of mental disorders.

Before schizophrenia specific clinical trials can begin, studies in animals will be necessary. “ the next important step.” Says Yoshikawa, “is to determine precisely which S1P receptor-acting drugs are effective in experimental animals. Although the new blockbuster drug fingolimod works at the S1P receptor and is effective at treating multiple sclerosis, we do not yet know effective it would be for schizophrenia.”
